Questions worth asking

- I have the context I need to make good decisions.
- I feel safe raising concerns or disagreeing.
- I'm growing in the ways that matter to me.
- The way we work makes it easier — not harder — to do great work.
- I would recommend this team as a great place to work.
Format
5-point Likert scale (strongly disagree → strongly agree), one optional free-text question at the end. Anonymous, run in the same tool every cycle so the trend is comparable.
The read-out
Share results within a week: scores, trend vs. last cycle, top themes from free-text, and two concrete actions you're taking. Silence after a survey is worse than not running one.
Common pitfalls
- — Long surveys. Response rates collapse past 12 questions.
- — Running the survey and not acting on it. That kills the next round's response rate.
- — Comparing to other teams instead of the team's own baseline.
